Upload Form

This PHP script just needs to be uploaded to your web server, or you can modify the appearance via CSS file.

Upload Form:

• contact form with file upload
• check form fields
• check validity of email address
• in the body of the email you will receive information about the uploaded file as well as a download link
• XHTML valid
• you can easily change the appearance and colors via CSS file
• you don't need a database for the script to work
