When you start the program, the mouse cursor turns into a window, which you just need to select the part of the screen you want to save in ICO, BMP or JPG format. You can freely resize the "capture" window, set your own background to combine with the captured one and define it, set the transparency of the image for the icon, ... You can also use an image from the gallery, where you can find more than 5000 images.
